Medical Records
Information regarding medical records can be obtained by calling our main office number: (318) 335-4881.
Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization from you.
Medical Records Release Authorization. You may fax the completed release form to our office at 318-335-4544.
Prescription Refills
If you need a refill for your medication, please call the pharmacy you use and ask them to fax over a refill request to (318) 335-4544. The refill requests will be done on a regular basis, based on doctor's approval. Please allow at least 72 hours (3 days) for processing the refill application.
